Please contact your insurance carrier/ Managed care entity and referring physician’s office three days before your scheduled appointment to ensure that pre-authorization has been obtained if required by your plan.  Your appointment might be cancelled if pre-authorization was required and not obtained prior to your scheduled procedure.
